Registration

We are so glad you have chosen to enroll your child into Solebury United Methodist Children's Learning Center. Many of our forms are required by the state of Pennsylvania.

Please go to the forms tab, print and complete all the forms needed to enroll your child/children. Complete and sign the registration checklist.

You have 60 days from the day of enrollment to have your child’s physical form completed and signed by a physician. This from will need to be completed yearly. All children and staff at Solebury United Methodist Children's Learning Center are required to have a yearly physical signed by a medical doctor on file. Children’s forms must include a copy of their vaccination record. If child is exempt from vaccines, the proper paper must also be on file.
